Project Summary

The Mountain States Genetics Regional Collaborative (MSGRC) developed plans to utilize family history as a tool to improve the identification and treatment of heritable conditions in the Mountain States region. Project objectives below were identified for the proposed Family Health History Project.

(1) identify validated family history tools and opportunities as well as barriers to their use;

(2) develop a strategic plan for regional implementation of electronic family history tools;

(3) develop a request for proposals (RFP) to facilitate introduction of these tools within practices; and

(4) fund four practices to pilot the use of family history tools in clinical practices in the Mountain States through the mini-grant mechanism if funding permits

In the Fall of 2014 a manuscript was developed that reviewed and validated family history tools and outlined potential implementation into primary care practices.
